As of 1 January 2016 Lasting Powers of Attorney (LPAs) can only be made using the new forms issued by the Office of the Public Guardian.

The new forms have been available for use since 1 July 2015 along with the old forms. It was possible to use either the new version or the old version of the LPA forms from 1 July 2015.

However, you can now no longer use the old forms to create an LPA.

Please note:

• Provided the old LPA forms have been completed, signed and dated properly by 1 January 2016 they can still be registered at any time so long as they have been made correctly.
